Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents including medication administration and treatments
• Monitor and document patient status, reporting changes promptly to the care team
• Perform skilled nursing tasks such as wound care, IV management (if applicable), and vital sign monitoring
• Collaborate with RNs, therapists, and support staff to create and execute person-centered care plans
• Educate residents and families on care plans and discharge instructions in a clear, respectful way
• Maintain accurate, timely charting in the facility's documentation system
• Promote a safe, clean environment and adhere to infection control and safety protocols

Qualifications

• Current LPN license in good standing (required)
• CPR/BLS certification (required) and willingness to obtain additional certifications if needed
• Previous experience in long-term care, skilled nursing, or similar clinical setting preferred
• Strong communication skills and the ability to work collaboratively on a multidisciplinary team
• Ability to manage time effectively during busy shifts and prioritize resident needs

Education

• Completion of an accredited LPN/Vocational Nursing program and required state licensure

Knowledge & Skills

• Solid medication administration and documentation skills
• Familiarity with electronic medical records and accurate charting practices
• Understanding of infection control, wound care, and basic nursing assessments
• Excellent interpersonal skills and the ability to build rapport with residents and families
• Ability to follow care plans and escalate clinical changes appropriately
