Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (Day); every other weekend coverage required
Hours per Week: 40
Candidate Pay Rate: $74 per hour

Roles & Responsibilities
- Coordinate patient-focused case management for pediatric populations from neonates through adolescents
- Lead discharge planning and care coordination to ensure safe, timely transitions back home or to the next level of care
- Collaborate with interdisciplinary teams (MDs, social work, therapy, and community resources) to optimize outcomes
- Maintain accurate and timely documentation in required charting systems
- Advocate for patients and families, identifying barriers to care and arranging appropriate resources
- Float to other UCSF facilities as needed, providing consistent, high-quality case management across sites
- Manage high patient ratios efficiently while prioritizing safety and quality of care
- Participate in quality improvement and unit-based initiatives to enhance pediatric care delivery
Qualifications
- Active CA RN License (required)
- AHA BLS (required)
- Minimum 2 years pediatric case management experience (required)
- Experience with discharge planning and care coordination (2 years preferred/required per posting)
- Trauma Level II experience (required); Trauma Level I experience preferred
- Travel experience required (comfortable working in multiple facilities)
- Proficiency with charting systems (required)
- Master's in Nursing or healthcare-related field (preferred)
- COVID-19 vaccine plus booster required per facility guideline (medical/religious exemptions only)
Education
- Master's in Nursing or healthcare-related field (preferred)
Knowledge & Skills
- Deep knowledge of pediatric case management and discharge planning
- Skilled in care coordination across hospital and community resources
- Comfortable working with neonatal, infant, pediatric, and adolescent patient populations
- Experience in acute hospital settings and familiarity with ICU/NICU/PICU environments
- Ability to manage higher patient ratios while maintaining quality and safety
- Strong documentation and charting system skills
- Collaborative communication with interdisciplinary teams and families
- Adaptability to float between UCSF facilities and rapidly changing unit needs
