Roles & Responsibilities
- Provide compassionate bedside care and support daily living needs
- Administer medications and treatments accurately and safely
- Monitor patient status and report changes promptly to the care team
- Collaborate with nurses, therapists and providers to deliver coordinated care
- Document assessments, interventions and patient responses in the chart
- Assist with admissions, transfers and discharges with professionalism
- Promote a safe, clean environment and follow infection control best practices
Qualifications
- Current LPN or RN license (as applicable) in good standing
- Demonstrated clinical experience in a long-term care or rehab setting preferred
- Basic Life Support/CPR certification or equivalent
- Ability to pass background check, drug screen and meet facility onboarding requirements
- Strong communication skills and a patient-centered attitude
Education
- Graduate of an accredited LPN program or approved nursing program
Knowledge & Skills
- Patient assessment and basic clinical observation
- Medication administration and documentation
- Wound care basics and restorative nursing principles
- Electronic medical record (EMR) familiarity and accurate charting
- Teamwork, time management and prioritization under typical nursing pressures
- Infection control practices and safe patient handling
