Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in a skilled nursing and rehabilitation setting
• Administer medications and treatments accurately, following facility policies and physician orders
• Monitor patient status and report changes promptly to the interdisciplinary team
• Document care thoroughly and clearly in the electronic medical record
• Assist with admission, transfer and discharge processes to ensure smooth transitions
• Support rehabilitation goals by collaborating with therapists and other caregivers
• Promote a safe, clean environment and follow infection prevention protocols
• Advocate for residents and communicate effectively with families and staff

Qualifications

• Active LPN or RN license in Massachusetts or the ability to obtain state licensure
• Current BLS or CPR certification required; additional certifications encouraged
• Comfortable working the night shift, demonstrating reliability and strong clinical judgment
• Excellent communication and interpersonal skills with a patient-first attitude
• Ability to perform physical aspects of the job, including safe patient handling and mobility assistance
• Previous experience in skilled nursing, rehab or long-term care is a plus

Education

• LPN: Practical nursing diploma or certificate
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)

Knowledge & Skills

• Strong medication administration and documentation skills
• Knowledge of wound care, IV basics for RNs as applicable, and post-acute rehab principles
• Proficiency with electronic medical records and charting
• Skilled at clinical assessment, prioritization and delegation
• Familiarity with infection control practices and safety standards
• Teamwork, problem solving and compassionate caregiving
