Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents in a skilled nursing and rehabilitation setting
- Administer medications and treatments accurately, following facility policies and physician orders
- Monitor patient status and report changes promptly to the interdisciplinary team
- Document care thoroughly and clearly in the electronic medical record
- Assist with admission, transfer and discharge processes to ensure smooth transitions
- Support rehabilitation goals by collaborating with therapists and other caregivers
- Promote a safe, clean environment and follow infection prevention protocols
- Advocate for residents and communicate effectively with families and staff
Qualifications
- Active LPN or RN license in Massachusetts or the ability to obtain state licensure
- Current BLS or CPR certification required; additional certifications encouraged
- Comfortable working the night shift, demonstrating reliability and strong clinical judgment
- Excellent communication and interpersonal skills with a patient-first attitude
- Ability to perform physical aspects of the job, including safe patient handling and mobility assistance
- Previous experience in skilled nursing, rehab or long-term care is a plus
Education
- LPN: Practical nursing diploma or certificate
- RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
Knowledge & Skills
- Strong medication administration and documentation skills
- Knowledge of wound care, IV basics for RNs as applicable, and post-acute rehab principles
- Proficiency with electronic medical records and charting
- Skilled at clinical assessment, prioritization and delegation
- Familiarity with infection control practices and safety standards
- Teamwork, problem solving and compassionate caregiving
