Shift Type: Night shift (NOC), 11:00 PM - 7:00 AM, days per facility schedule
Hours per Week: 40
Candidate Pay Rate: $42 per hour

Roles & Responsibilities
- Provide compassionate direct patient care for residents in a skilled nursing/rehab setting
- Administer medications and treatments accurately and document per facility policy
- Monitor and report changes in patient condition to the care team promptly
- Collaborate with nurses, therapists, and providers to create and follow individualized care plans
- Support admissions, discharges, and transfers to ensure seamless resident transitions
- Educate residents and families about care plans and health goals in a respectful manner
- Maintain timely, accurate charting and participate in quality improvement efforts
Qualifications
- Current LPN or RN license valid in Massachusetts (required)
- Basic Life Support (BLS) certification (or willingness to obtain upon hire)
- Comfortable working night shifts in a long-term care/rehab environment
- Excellent communication skills and a team-oriented mindset
- Prior experience in skilled nursing, rehab, or long-term care is preferred but new grads with strong clinical training are welcome to apply
Education
- Graduate of an accredited Practical Nursing or Registered Nursing program
- High school diploma or GED required
Knowledge & Skills
- Strong clinical assessment and monitoring skills
- Medication administration and accurate documentation practices
- Familiarity with wound care, ADL support, and routine nursing procedures
- Ability to prioritize, multitask, and make sound clinical judgments at night
- Comfortable using electronic health records and following facility protocols
