Roles & Responsibilities

• Provide compassionate direct patient care for residents in a skilled nursing/rehab setting
• Administer medications and treatments accurately and document per facility policy
• Monitor and report changes in patient condition to the care team promptly
• Collaborate with nurses, therapists, and providers to create and follow individualized care plans
• Support admissions, discharges, and transfers to ensure seamless resident transitions
• Educate residents and families about care plans and health goals in a respectful manner
• Maintain timely, accurate charting and participate in quality improvement efforts

Qualifications

• Current LPN or RN license valid in Massachusetts (required)
• Basic Life Support (BLS) certification (or willingness to obtain upon hire)
• Comfortable working night shifts in a long-term care/rehab environment
• Excellent communication skills and a team-oriented mindset
• Prior experience in skilled nursing, rehab, or long-term care is preferred but new grads with strong clinical training are welcome to apply

Education

• Graduate of an accredited Practical Nursing or Registered Nursing program
• High school diploma or GED required

Knowledge & Skills

• Strong clinical assessment and monitoring skills
• Medication administration and accurate documentation practices
• Familiarity with wound care, ADL support, and routine nursing procedures
• Ability to prioritize, multitask, and make sound clinical judgments at night
• Comfortable using electronic health records and following facility protocols
