Shift Type: 7:00 AM - 3:00 PM (Day shift; specific days to be confirmed with hiring team)
Hours per Week: 40
Candidate Pay Rate: $42 per hour

Roles & Responsibilities
- Provide compassionate direct patient care in a skilled nursing and rehab setting
- Administer medications and treatments safely and following facility protocols
- Monitor and document patient vital signs, changes, and progress in the chart
- Collaborate with RNs, therapists, and interdisciplinary team members to support care plans
- Assist residents with activities of daily living, mobility, and comfort measures
- Communicate clearly and respectfully with residents and their families
- Maintain a safe, clean environment and follow infection control best practices
Qualifications
- Current Licensed Practical Nurse (LPN) license in Massachusetts (required)
- Current CPR/BLS certification (or willingness to obtain promptly)
- Comfortable working in long-term care/rehabilitation environments
- Strong communication skills and a team-first attitude
- Prior experience in a nursing home, rehab, or similar setting is a plus
Education
- Practical Nursing diploma or equivalent vocational training; additional certifications welcomed
Knowledge & Skills
- Medication administration and basic clinical assessments
- Charting and documentation (electronic medical record familiarity helpful)
- Wound care basics and pressure injury prevention
- Infection control practices and safe patient handling
- Ability to prioritize tasks and respond to changing resident needs
