Roles & Responsibilities
- Deliver compassionate bedside care to residents and patients, ensuring dignity and respect
- Administer medications and treatments safely and accurately according to orders
- Monitor patient status, record vital signs, and report meaningful changes promptly
- Collaborate with nurses, therapists, and the interdisciplinary team to support care plans
- Assist residents with activities of daily living and mobility needs
- Document nursing interventions clearly in the medical record to support continuity of care
- Support safe transitions of care and communicate effectively with families and providers
- Adhere to infection control, safety protocols, and facility policies at all times
Qualifications
- Current and valid LPN license (ability to practice in Massachusetts preferred)
- BLS/CPR certification (or willingness to obtain upon hire)
- Experience in long-term care, rehab, or skilled nursing preferred but new grads with strong clinical skills are welcome
- Ability to communicate clearly with patients, families, and team members
- Compassionate, patient-centered approach and strong attention to detail
Education
- Completion of an accredited LPN/LVN program and high school diploma or equivalent
Knowledge & Skills
- Medication administration and resident monitoring
- Clinical assessment and basic wound care knowledge
- Familiarity with electronic health records and documentation best practices
- Strong time management and prioritization under typical long-term care workflows
- Effective team communication and collaboration skills
