Roles & Responsibilities

• Administer medications and treatments safely and accurately according to physician orders
• Monitor and document patient vitals, status changes, and response to interventions
• Provide hands-on nursing care and assist residents with activities of daily living
• Perform wound care, dressing changes, and basic clinical procedures as needed
• Collaborate with the interdisciplinary care team to create and follow individualized care plans
• Communicate clearly and compassionately with residents and families, providing education and reassurance
• Escalate clinical concerns promptly to nurse leadership or providers to ensure resident safety
• Maintain accurate, timely documentation in the patient record and comply with facility policies

Qualifications

• Current, active LPN or RN license in Massachusetts (must be in good standing)
• Basic Life Support (BLS/CPR) certification required
• Comfortable working night shifts and providing compassionate care in a rehab/nursing setting
• Strong communication skills and ability to work as part of a collaborative team
• Reliable, professional, and committed to resident-centered care

Education

• LPN: Completion of an accredited practical nursing program (diploma/certificate)
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Medication administration and basic clinical assessment skills
• Familiarity with long-term care or skilled nursing/rehabilitation workflows
• Accurate charting and documentation practices
• Infection control practices and safe patient handling
• Time management and prioritization in a busy clinical environment
• Empathy, strong interpersonal skills, and patient-family communication
