Roles & Responsibilities
- Administer medications and treatments safely and accurately according to physician orders
- Monitor and document patient vitals, status changes, and response to interventions
- Provide hands-on nursing care and assist residents with activities of daily living
- Perform wound care, dressing changes, and basic clinical procedures as needed
- Collaborate with the interdisciplinary care team to create and follow individualized care plans
- Communicate clearly and compassionately with residents and families, providing education and reassurance
- Escalate clinical concerns promptly to nurse leadership or providers to ensure resident safety
- Maintain accurate, timely documentation in the patient record and comply with facility policies
Qualifications
- Current, active LPN or RN license in Massachusetts (must be in good standing)
- Basic Life Support (BLS/CPR) certification required
- Comfortable working night shifts and providing compassionate care in a rehab/nursing setting
- Strong communication skills and ability to work as part of a collaborative team
- Reliable, professional, and committed to resident-centered care
Education
- LPN: Completion of an accredited practical nursing program (diploma/certificate)
- RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred
Knowledge & Skills
- Medication administration and basic clinical assessment skills
- Familiarity with long-term care or skilled nursing/rehabilitation workflows
- Accurate charting and documentation practices
- Infection control practices and safe patient handling
- Time management and prioritization in a busy clinical environment
- Empathy, strong interpersonal skills, and patient-family communication
