Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
• Perform routine assessments and document changes in condition accurately and promptly
• Administer medications and treatments safely according to orders and facility policies
• Collaborate with the interdisciplinary care team to create and follow individualized care plans
• Communicate clearly with families and team members to ensure continuity of care
• Maintain a clean, safe environment and follow infection control and safety protocols

Qualifications

• Active LPN license in Massachusetts (or eligible for licensure)
• Current CPR/BLS certification (or ability to obtain upon hire)
• Comfortable working night shifts and functioning independently during assigned hours
• Strong communication, documentation, and patient-assessment skills
• Previous experience in long-term care, rehab, or skilled nursing settings is a plus but not required

Education

• Graduate of an accredited practical nursing program (LPN) or accredited nursing program (if applying as RN)
• State licensure required for the role

Knowledge & Skills

• Knowledge of medication administration and basic nursing procedures
• Ability to perform resident assessments and recognize changes in condition
• Familiarity with electronic documentation or paper charting systems
• Effective communication and teamwork skills in a fast-paced clinical setting
• Understanding of infection control, patient safety, and resident rights
