Roles & Responsibilities

• Provide compassionate, person-centered nursing care to residents in a skilled nursing/rehab setting
• Administer medications and treatments accurately and document care in the medical record
• Perform thorough assessments and communicate changes in condition to the care team
• Collaborate with interdisciplinary staff to develop and follow individualized care plans
• Support residents with activities of daily living while promoting dignity and independence
• Ensure compliance with facility policies, infection control, and safety protocols
• Participate in shift handoffs and contribute to a positive, team-focused environment

Qualifications

• Active LPN or RN license in Massachusetts (required)
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Experience in long-term care, skilled nursing, or rehab is a strong plus
• Comfortable with medication administration, charting, and clinical assessments
• Strong communication skills and a collaborative attitude

Education

• LPN: Completion of an accredited Practical Nursing program (diploma or certificate)
• RN: Associate Degree in Nursing (ADN) or Diploma in Nursing (as applicable for RN applicants)

Knowledge & Skills

• Solid knowledge of nursing fundamentals and medication administration
• Familiarity with electronic charting and documentation best practices
• Ability to prioritize tasks in a busy night-shift environment
• Good clinical judgement and the ability to escalate concerns appropriately
• Compassionate patient interaction and strong teamwork skills
