Roles & Responsibilities

• Provide compassionate bedside care and monitor residents with respect and dignity
• Administer medications and treatments accurately and document care thoroughly
• Assess and record vital signs, changes in condition, and communicate concerns promptly
• Support rehabilitative care and help implement individualized care plans
• Collaborate with the interdisciplinary team to deliver coordinated, high-quality care
• Maintain clear, timely charting and follow facility policies for safety and infection control
• Assist with admissions, transfers, and discharge planning when needed
• Mentor and support nursing assistants and new team members during shifts

Qualifications

• Current LPN license in Massachusetts required (RN considered where applicable)
• Active BLS/CPR certification preferred or obtained upon hire
• Comfort with long-term care or rehabilitation populations is a plus
• Strong communication skills and the ability to work well in a team
• Valid, up-to-date immunizations and background checks per facility policy

Education

• Completion of an accredited practical nursing program (LPN) or registered nursing program (RN)
• High school diploma or GED required

Knowledge & Skills

• Medication administration and documentation best practices
• Wound care basics and pressure injury prevention
• Skilled in resident assessment, ADL support, and care planning
• Familiarity with electronic medical records and accurate charting
• Ability to prioritize, triage, and escalate changes in patient condition
• Professional bedside manner and strong interpersonal skills
