Roles & Responsibilities
- Provide high-quality physical therapy evaluations and individualized treatment plans for residents in a skilled nursing facility
- Deliver skilled interventions including therapeutic exercise, gait training, balance and functional mobility work
- Document progress clearly and timely in the facility’s medical record system
- Collaborate with nursing, therapy colleagues, and interdisciplinary teams to optimize resident outcomes
- Educate residents and families on home programs, safety, and discharge planning
- Participate in care planning and meetings to ensure coordinated, person-centered care
Qualifications
- Doctor of Physical Therapy (DPT) or PT credential — facility accepts new grads (any experience, including new grad, is acceptable)
- Active Physical Therapy license in Illinois (or ability to obtain prior to start)
- Basic Life Support (BLS/CPR) certification (facility may require upon hire)
- Comfortable working in a skilled nursing facility (SNF) setting
- Strong communication and documentation skills; team-player attitude
Education
- Doctor of Physical Therapy (DPT) preferred; new grads welcome

- Skilled nursing facility rehab practices and resident-focused care
- Clinical assessment and therapeutic intervention planning
- Gait training, transfers, balance and functional mobility strategies
- Clear documentation and charting for regulatory compliance
- Interdisciplinary collaboration and effective communication with families
