Roles & Responsibilities

• Provide high-quality physical therapy evaluations and individualized treatment plans for residents in a skilled nursing facility
• Deliver skilled interventions including therapeutic exercise, gait training, balance and functional mobility work
• Document progress clearly and timely in the facility’s medical record system
• Collaborate with nursing, therapy colleagues, and interdisciplinary teams to optimize resident outcomes
• Educate residents and families on home programs, safety, and discharge planning
• Participate in care planning and meetings to ensure coordinated, person-centered care

Qualifications

• Doctor of Physical Therapy (DPT) or PT credential — facility accepts new grads (any experience, including new grad, is acceptable)
• Active Physical Therapy license in Illinois (or ability to obtain prior to start)
• Basic Life Support (BLS/CPR) certification (facility may require upon hire)
• Comfortable working in a skilled nursing facility (SNF) setting
• Strong communication and documentation skills; team-player attitude

Education

• Doctor of Physical Therapy (DPT) preferred; new grads welcome

• Skilled nursing facility rehab practices and resident-focused care
• Clinical assessment and therapeutic intervention planning
• Gait training, transfers, balance and functional mobility strategies
• Clear documentation and charting for regulatory compliance
