Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to assigned residents and patients
- Administer medications and treatments safely and accurately according to orders
- Monitor and document patient status, vital signs, and changes in condition in the medical record
- Collaborate with the interdisciplinary care team to create and follow individualized care plans
- Support patients and families through education, communication, and emotional support
- Assist with admissions, transfers, and discharge planning to ensure smooth transitions
Qualifications
- Current Licensed Practical Nurse (LPN) license in Pennsylvania (required)
- Active basic life support (BLS/CPR) certification preferred or ability to obtain
- Clinical experience as an LPN in long-term care, rehab, or acute care settings preferred
- Strong communication skills and the ability to work well within a care team
- Ability to perform duties required for overnight shifts, including medication administration and documentation
Education
- Graduate of an accredited Licensed Practical Nurse (LPN) program or equivalent
Knowledge & Skills
- Solid knowledge of nursing fundamentals and medication administration
- Comfortable using electronic medical records and documentation systems
- Good clinical assessment skills and attention to detail
- Infection control practices and safe patient handling
- Strong interpersonal skills for patient and family education
