Roles & Responsibilities
- Ensure continuity of care by providing direct clinical nursing and supervising CNAs, LVNs and support staff
- Assess, plan, implement and evaluate individualized nursing care for residents in the skilled nursing setting
- Communicate promptly and clearly with physicians and the interdisciplinary health team about changes in resident condition
- Respond quickly and accurately to changes in clinical condition and to treatment responses
- Lead and mentor staff as the charge nurse to promote high-quality, compassionate care
- Participate actively in Quality Improvement activities to enhance resident outcomes
- Maintain accurate documentation in the facility record and support Electronic Medical Records workflows
- Interface professionally and respectfully with residents, families, staff and regulatory representatives
Qualifications
- Current California RN license (required)
- Minimum two (2) years prior experience in a long-term care/skilled nursing facility
- Current BLS certification (required)
- Working knowledge of Title 22, OBRA and long-term care regulations
- Supervisory or charge nurse experience preferred
- Ability to pass the nursing medical test prior to assignment
- Proficient English communication skills, both verbal and written
- Basic computer skills; EMR experience is a plus
Education
- Active California Registered Nurse (RN) license (required)
- Current BLS certification
Knowledge & Skills
- Patient assessment, care planning, implementation and evaluation
- Supervising CNAs, LVNs, RNs and support staff with strong leadership
- Clear communication with physicians and multidisciplinary teams
- Quality Improvement participation and regulatory compliance
- Familiarity with Electronic Medical Records
- Practical knowledge of Title 22 and OBRA regulations
