Roles & Responsibilities

• Ensure continuity of care by providing direct clinical nursing and supervising CNAs, LVNs and support staff
• Assess, plan, implement and evaluate individualized nursing care for residents in the skilled nursing setting
• Communicate promptly and clearly with physicians and the interdisciplinary health team about changes in resident condition
• Respond quickly and accurately to changes in clinical condition and to treatment responses
• Lead and mentor staff as the charge nurse to promote high-quality, compassionate care
• Participate actively in Quality Improvement activities to enhance resident outcomes
• Maintain accurate documentation in the facility record and support Electronic Medical Records workflows
• Interface professionally and respectfully with residents, families, staff and regulatory representatives

Qualifications

• Current California RN license (required)
• Minimum two (2) years prior experience in a long-term care/skilled nursing facility
• Current BLS certification (required)
• Working knowledge of Title 22, OBRA and long-term care regulations
• Supervisory or charge nurse experience preferred
• Ability to pass the nursing medical test prior to assignment
• Proficient English communication skills, both verbal and written
• Basic computer skills; EMR experience is a plus

Education

• Active California Registered Nurse (RN) license (required)
• Current BLS certification

Knowledge & Skills

• Patient assessment, care planning, implementation and evaluation
• Supervising CNAs, LVNs, RNs and support staff with strong leadership
• Clear communication with physicians and multidisciplinary teams
• Quality Improvement participation and regulatory compliance
• Familiarity with Electronic Medical Records
• Practical knowledge of Title 22 and OBRA regulations
