Roles & Responsibilities

• Provide compassionate direct patient care and assistance with daily living activities
• Monitor and record vital signs, reporting any changes to the nursing team promptly
• Assist with safe transfers, ambulation and mobility to promote patient independence
• Support residents with personal hygiene, feeding and toileting with dignity
• Document patient interactions and care accurately in the chart
• Collaborate with nurses, therapists and family members to deliver person centered care
• Maintain a safe and clean patient environment and follow infection control protocols

Qualifications

• Active Certified Nursing Assistant (CNA) credential required
• Current BLS/CPR certification preferred or willing to obtain
• Comfortable working night shifts and supporting residents with varied needs
• Strong communication skills and ability to work well within a team
• Background check and health screening clearance as required by facility

Education

• High school diploma or GED preferred
• Completion of an approved CNA training program and state certification

Knowledge & Skills

• Patient handling and safe transfer techniques
• Basic clinical skills: vitals, feeding, toileting, hygiene assistance
• Familiarity with infection control and standard precautions
• Good documentation practices and attention to detail
• Compassionate bedside manner and strong interpersonal skills
• Ability to follow care plans and communicate observations to nursing staff
