Roles & Responsibilities

• Assess patient mobility, strength, balance and function using evidence-based evaluations
• Develop individualized treatment plans focused on restoring function and independence
• Implement therapeutic interventions including manual therapy, therapeutic exercise and gait training
• Document progress clearly and timely in the electronic health record to support continuity of care
• Collaborate with interdisciplinary teams to coordinate care and optimize patient outcomes
• Educate patients and caregivers on home programs and injury prevention strategies
• Monitor and adjust treatment plans based on patient response and goals
• Contribute to a positive clinic culture by mentoring colleagues and sharing best practices

Qualifications

• Active California Physical Therapist license required
• Doctor of Physical Therapy (DPT) or Master's in Physical Therapy preferred
• Current BLS/CPR certification required or willing to obtain promptly
• Clinical experience in outpatient and/or contract settings preferred
• Strong documentation skills and familiarity with mainstream EHR systems
• Excellent communication skills and a patient-centered approach

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy or related field considered

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise and manual therapy techniques
• Knowledge of mobility training, balance retraining and assistive device recommendations
• Proficient with clinical documentation and outcome measures
• Ability to develop measurable goals and track progress objectively
• Strong interpersonal skills for patient and team collaboration
• Comfortable adapting interventions for a diverse patient population
