Roles & Responsibilities
- Assess patient mobility, strength, balance and function using evidence-based evaluations
- Develop individualized treatment plans focused on restoring function and independence
- Implement therapeutic interventions including manual therapy, therapeutic exercise and gait training
- Document progress clearly and timely in the electronic health record to support continuity of care
- Collaborate with interdisciplinary teams to coordinate care and optimize patient outcomes
- Educate patients and caregivers on home programs and injury prevention strategies
- Monitor and adjust treatment plans based on patient response and goals
- Contribute to a positive clinic culture by mentoring colleagues and sharing best practices
Qualifications
- Active California Physical Therapist license required
- Doctor of Physical Therapy (DPT) or Master's in Physical Therapy preferred
- Current BLS/CPR certification required or willing to obtain promptly
- Clinical experience in outpatient and/or contract settings preferred
- Strong documentation skills and familiarity with mainstream EHR systems
- Excellent communication skills and a patient-centered approach
Education
- Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy or related field considered
Knowledge & Skills
- Skilled in patient assessment, therapeutic exercise and manual therapy techniques
- Knowledge of mobility training, balance retraining and assistive device recommendations
- Proficient with clinical documentation and outcome measures
- Ability to develop measurable goals and track progress objectively
- Strong interpersonal skills for patient and team collaboration
- Comfortable adapting interventions for a diverse patient population
