Roles & Responsibilities

• Provide compassionate, person-centered care to residents, helping them feel safe and respected
• Assist with activities of daily living including bathing, dressing, grooming, toileting, and mobility
• Take and record vital signs accurately and report any concerning changes to the nurse
• Support mealtime needs and follow dietary plans as directed
• Maintain a clean and safe environment, following infection control and safety procedures
• Document care and resident interactions clearly and promptly in the chart
• Communicate effectively with residents, families, and the care team to ensure continuity of care
• Participate in care team huddles and contribute to quality improvement efforts

Qualifications

• Current Certified Nursing Assistant (CNA) certification valid for Pennsylvania
• CPR/BLS certification preferred or willing to obtain upon hire
• Excellent communication skills and a compassionate, patient-first attitude
• Ability to perform physical aspects of the role including assisting with transfers and mobility
• Background check and any required health screenings completed per facility policy

Education

• High school diploma or GED preferred
• Current CNA certification required

Knowledge & Skills

• Familiarity with basic nursing procedures and resident care routines
• Comfortable taking and documenting vital signs and observing clinical changes
• Good infection control practices and safe body mechanics
• Strong interpersonal skills and teamwork mindset
• Basic charting skills; willingness to learn facility electronic documentation if used
