Roles & Responsibilities

• Provide compassionate direct patient care, including daily assessments and bedside support
• Administer medications and treatments accurately and document appropriately
• Collaborate with RNs, providers, and interdisciplinary team members to support care plans
• Monitor patient status, identify changes promptly, and escalate concerns when needed
• Maintain clear, timely charting in the electronic medical record
• Support residents' activities of daily living with dignity and respect
• Participate in team huddles, safety initiatives, and quality-improvement efforts

Qualifications

• Current Licensed Practical Nurse (LPN) license — required
• Basic Life Support (BLS/CPR) certification or willingness to obtain upon hire
• Comfortable with medication administration, wound care basics, and routine assessments
• Strong communication skills and a caring bedside manner
• Previous experience in long-term care, skilled nursing, or similar settings preferred

Education

• Vocational/Practical Nursing diploma or equivalent

Knowledge & Skills

• Solid knowledge of vital sign monitoring, ADL assistance, and infection control
• Familiarity with medication administration and documentation best practices
• Good charting skills and comfort using electronic medical records
• Ability to prioritize tasks in a fast-paced environment while maintaining attention to detail
• Teamwork, empathy, and clear communication with residents and families
