Roles & Responsibilities
- Conduct timely, comprehensive assessments of speech, language, cognition, and swallowing — often within 24 hours of physician referral
- Develop functional therapy diagnoses and measurable prognoses tailored to each resident
- Design and implement individualized, evidence‑based treatment plans and adjust them as residents progress
- Deliver direct therapy for communication and dysphagia, including compensatory strategies and cognitive‑communication interventions
- Lead and document group therapy sessions and create clear, timely clinical documentation (evals, daily notes, weekly updates, 14‑day physician reports, discharge summaries)
- Educate residents, families, and nursing/CNA staff on communication strategies and safe swallowing techniques
- Coordinate care with nurses, physicians, PT/OT, dietary, and social work to optimize outcomes and support discharge planning
- Participate in quality improvement initiatives and ensure compliance with Medicare/Medicaid documentation standards (PDPM, Title 22, CMS)
Qualifications
- Master's degree in Speech‑Language Pathology from an accredited institution (required)
- Current California SLP license (temporary RPE accepted if active application in process)
- BLS/CPR certification (current) — required
- Certificate of Clinical Competence (CCC‑SLP) from ASHA preferred
- Experience in skilled nursing facility, geriatric, or rehabilitation settings preferred
- If supervising SLPAs: at least 2 years (or 3,120 hours) of experience and familiarity with CA supervision rules
- Strong clinical assessment, treatment planning, documentation, and communication skills
Education
- Master's degree in Speech‑Language Pathology (required)
Knowledge & Skills
- Skilled in dysphagia assessment and management and safe swallowing strategies
- Experience with cognitive‑communication therapy and stroke/dementia populations
- Thorough knowledge of Medicare/Medicaid documentation, PDPM, Title 22, and CMS requirements
- Proficiency in timely, compliant documentation (evaluations, progress notes, POC logs, billing docs)
- Excellent interdisciplinary collaboration and discharge planning abilities
- Ability to manage clinical productivity and scheduling while maintaining high quality care
- Comfort leading patient/family education and group therapy sessions
