Roles & Responsibilities

• Provide high-quality, compassionate nursing care to residents at Bel-Aire Center
• Administer medications and treatments accurately and safely
• Perform thorough patient assessments and document findings in the chart
• Collaborate with interdisciplinary teams to develop and follow individualized care plans
• Support residents with activities of daily living while promoting independence
• Communicate clearly with residents, families, and staff to ensure continuity of care
• Respond to changes in resident condition and escalate concerns appropriately

Qualifications

• Active Licensed Practical Nurse (LPN) license in Vermont (required)
• Current Basic Life Support (BLS) certification (or ability to obtain prior to start)
• Experience in long-term care, rehabilitation, or similar clinical settings preferred
• Strong documentation skills and comfort using electronic medical records
• Excellent communication, teamwork, and resident-centered care approach

Education

• Completed accredited Practical Nursing program (diploma or certificate)

Knowledge & Skills

• Medication administration and safe documentation practices
• Wound care basics and monitoring/ reporting of changes
• Vital signs assessment and routine clinical monitoring
• Familiarity with long-term care regulations and resident rights
• Good clinical judgment, time management, and teamwork
