Roles & Responsibilities
- Provide high-quality, compassionate nursing care to residents at Bel-Aire Center
- Administer medications and treatments accurately and safely
- Perform thorough patient assessments and document findings in the chart
- Collaborate with interdisciplinary teams to develop and follow individualized care plans
- Support residents with activities of daily living while promoting independence
- Communicate clearly with residents, families, and staff to ensure continuity of care
- Respond to changes in resident condition and escalate concerns appropriately
Qualifications
- Active Licensed Practical Nurse (LPN) license in Vermont (required)
- Current Basic Life Support (BLS) certification (or ability to obtain prior to start)
- Experience in long-term care, rehabilitation, or similar clinical settings preferred
- Strong documentation skills and comfort using electronic medical records
- Excellent communication, teamwork, and resident-centered care approach
Education
- Completed accredited Practical Nursing program (diploma or certificate)
Knowledge & Skills
- Medication administration and safe documentation practices
- Wound care basics and monitoring/ reporting of changes
- Vital signs assessment and routine clinical monitoring
- Familiarity with long-term care regulations and resident rights
- Good clinical judgment, time management, and teamwork
