Roles & Responsibilities
- Provide compassionate, patient-centered nursing care for long-term care residents
- Administer medications, document treatments, and monitor responses safely and accurately
- Perform routine assessments and record vital signs, reporting changes in condition promptly
- Support residents with activities of daily living while promoting dignity and independence
- Collaborate with RNs, CNAs and interdisciplinary team members to deliver coordinated care
- Maintain accurate, timely documentation in the facility charting system
- Follow infection control protocols and facility policies to ensure resident safety
- Educate residents and families about care plans, treatments and discharge instructions
Qualifications
- Current LPN license (Licensed Practical Nurse) eligible in Vermont
- CPR/BLS certification (or willingness to obtain prior to start)
- Comfortable working 12+ hour day shifts
- Experience in long-term care, skilled nursing, or geriatrics preferred
- Strong clinical assessment and medication administration skills
- Reliable, professional, and able to adapt to a fast-paced facility environment
Education
- Graduate of an accredited practical nursing program (LPN diploma or certificate)
Knowledge & Skills
- Medication administration and documentation best practices
- Resident assessment, wound care basics, and chronic condition management
- Familiarity with electronic medical records and accurate charting
- Excellent communication and team collaboration skills
- Time management and prioritization across multiple residents
- Infection prevention and standard precautions knowledge
