Roles & Responsibilities

• Provide compassionate, patient-centered nursing care for long-term care residents
• Administer medications, document treatments, and monitor responses safely and accurately
• Perform routine assessments and record vital signs, reporting changes in condition promptly
• Support residents with activities of daily living while promoting dignity and independence
• Collaborate with RNs, CNAs and interdisciplinary team members to deliver coordinated care
• Maintain accurate, timely documentation in the facility charting system
• Follow infection control protocols and facility policies to ensure resident safety
• Educate residents and families about care plans, treatments and discharge instructions

Qualifications

• Current LPN license (Licensed Practical Nurse) eligible in Vermont
• CPR/BLS certification (or willingness to obtain prior to start)
• Comfortable working 12+ hour day shifts
• Experience in long-term care, skilled nursing, or geriatrics preferred
• Strong clinical assessment and medication administration skills
• Reliable, professional, and able to adapt to a fast-paced facility environment

Education

• Graduate of an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

• Medication administration and documentation best practices
• Resident assessment, wound care basics, and chronic condition management
• Familiarity with electronic medical records and accurate charting
• Excellent communication and team collaboration skills
• Time management and prioritization across multiple residents
• Infection prevention and standard precautions knowledge
