Roles & Responsibilities
- Conduct thorough patient evaluations to identify functional goals and needs
- Develop and implement individualized treatment plans focused on meaningful progress
- Provide daily treatments that help patients regain mobility, independence, and confidence
- Manage timely and safe discharges with clear home and follow-up instructions
- Document patient progress accurately and communicate updates with the rehab team
- Collaborate with nurses, aides, and therapy colleagues to deliver coordinated care
- Maintain productivity expectations and follow CRG policies and procedures
Qualifications
- PT or DPT credential (DPT listed as a primary skill)
- 1–3 years of clinical experience (listed requirement)
- Prior SNF experience strongly preferred
- Willingness to follow facility dress code (black scrubs) and CRG policies
- Comfort working on-site (therapist must be present in the building to receive guaranteed hours)
Education
- Doctor of Physical Therapy (DPT) or equivalent PT degree preferred
Knowledge & Skills
- Skilled in patient evaluations and creating goal-driven treatment plans
- Experience providing daily treatments and managing discharges in a rehabilitation setting
- Strong documentation and communication skills to keep the interdisciplinary team aligned
- Ability to meet productivity expectations while delivering high-quality patient care
- Familiarity with SNF workflows and patient populations is a strong plus
