Roles & Responsibilities
- Provide compassionate, skilled physical therapy interventions appropriate for a skilled nursing facility population
- Assist with patient assessments, mobility training, transfers, gait and balance programs
- Collaborate closely with nurses, therapists and interdisciplinary team members to optimize patient outcomes
- Complete timely, accurate documentation and contribute to patient care plans
- Adhere to Continuum Rehabilitation Group policies and facility procedures
- Maintain productivity standards (facility notes a 92% productivity expectation)
- Represent the therapy team professionally (black scrubs dress code) and be present onsite to receive guaranteed hours
Qualifications
- 1–3 years of relevant PTA experience (as noted in the job posting)
- Credentialed as a PTA per facility/state requirements (have documentation ready)
- Experience working in a skilled nursing facility (SNF) or similar post-acute care setting preferred
- Ability to follow facility policies and meet established productivity expectations
- Reliable, able to be present onsite during scheduled hours
Education

- Hands-on experience with mobility, gait training and therapeutic exercise for older adults
- Familiarity with SNF documentation and care planning
