Roles & Responsibilities

• Provide direct, compassionate nursing care to residents as an LPN, ensuring dignity and safety
• Administer medications and treatments accurately and in a timely manner
• Monitor and document vital signs, changes in condition, and resident responses
• Collaborate with RNs, physicians, and interdisciplinary team members to coordinate care
• Perform wound care, catheter management, and other clinical procedures as required
• Educate residents and families about care plans, procedures, and medications
• Maintain accurate, timely charting and follow infection-control and facility policies

Qualifications

• Current LPN license (Pennsylvania) — required
• BLS/CPR certification — required or able to obtain prior to start
• Comfortable with night shift work and rotating schedules (every other weekend required)
• Ability to pass background check, drug screen, and meet facility onboarding requirements
• Previous experience in long-term care, skilled nursing, or a related setting is a strong plus

Education

• Completed Practical Nursing program (LPN diploma or equivalent)

Knowledge & Skills

• Medication administration and safe documentation practices
• Skilled in resident assessment and recognizing changes in condition
• Strong clinical judgment and ability to follow care plans
• Effective communication and teamwork with clinical staff and families
• Basic wound care, catheter care, and infection prevention techniques
• Time management skills to prioritize tasks during busy night shifts
