Roles & Responsibilities

• Provide compassionate direct patient care, including medication administration and routine nursing tasks
• Perform focused assessments and monitor patient status throughout the shift
• Document accurate and timely charting in the electronic medical record
• Collaborate with RNs, providers, and the care team to promote patient safety and continuity of care
• Assist with activities of daily living and support patient comfort and dignity
• Communicate clearly with patients and families to educate and set expectations
• Escalate changes in patient condition promptly and follow facility protocols

Qualifications

• Current and active LPN license (Licensed Practical Nurse) — New Mexico license strongly preferred
• Basic Life Support (BLS) / CPR certification required
• Minimum clinical experience as an LPN; prior long-term care or behavioral health experience is a plus
• Ability to work overnight shifts (6:00 PM - 6:30 AM) and adapt to a travel assignment
• Up-to-date immunizations and the ability to pass background and health screenings
• Reliable, professional, and able to integrate quickly into an established team

Education

• LPN diploma or vocational nursing certificate required
• Current LPN license (state-specific) required

Knowledge & Skills

• Strong fundamentals in medication administration and vital signs monitoring
• Comfortable with documentation and electronic medical records
• Patient-centered communication and teamwork skills
• Good clinical judgment and ability to follow care plans
• Time management and the ability to handle multiple priorities on busy shifts
• Basic wound care and ADL assistance knowledge
