Roles & Responsibilities

• Provide compassionate, hands-on direct patient care to meet daily needs
• Administer medications and treatments accurately and safely as ordered
• Monitor patient status and report changes promptly to the care team
• Document care thoroughly and maintain accurate patient records
• Collaborate with nurses, therapists, and physicians to implement care plans
• Educate patients and families about treatments, medications, and aftercare
• Support infection control and safety protocols to protect patients and staff

Qualifications

• Active LPN (Licensed Practical Nurse) license; T-LPN/travel LPN credential or eligibility welcomed
• Current Basic Life Support (BLS/CPR) certification preferred or obtained upon hire
• Comfortable working 12+ hour day shifts and managing a full patient assignment
• Strong communication skills and a team-oriented mindset
• Current immunizations and ability to meet facility onboarding requirements

Education

• Graduate of an accredited LPN/Vocational Nursing program

Knowledge & Skills

• Skilled in medication administration and basic clinical procedures
• Proficient in patient assessment, vital signs, and documentation
• Able to prioritize tasks in a fast-paced clinical setting
• Familiarity with electronic health records and charting best practices
• Strong interpersonal skills for patient/family education and teamwork
