Roles & Responsibilities

• Evaluate patients to determine functional status, needs, and goals
• Develop and implement individualized treatment plans that restore mobility and independence
• Provide skilled therapeutic interventions including gait training, transfers, and balance retraining
• Document progress and outcomes clearly and in compliance with facility and regulatory standards
• Collaborate with nurses, OT, SLP, and interdisciplinary team to optimize patient discharge outcomes
• Educate patients and families on home exercise programs and safety strategies
• Advocate for patient-centered care and contribute to quality improvement initiatives within the SNF

Qualifications

• Licensed Physical Therapist in Ohio (or eligible for Ohio licensure)
• DPT preferred; open to clinicians with a PT degree — new grads welcome (job allows any experience, including new grad)
• Comfortable working in a Skilled Nursing Facility (SNF) setting
• Strong clinical reasoning, documentation skills, and the ability to collaborate with an interdisciplinary team
• Current active professional credentials and ability to meet facility onboarding requirements

Education

• DPT (Doctor of Physical Therapy) or equivalent Physical Therapy degree

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, and mobility/transfer training
• Experience or comfort with SNF-level care and post-acute rehabilitation needs
• Knowledge of safe patient handling and fall prevention strategies
• Proficient documentation and care planning to meet regulatory and payer requirements
• Ability to communicate clearly with patients, families, and clinical teams
