Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents in line with care plans
- Administer medications and treatments accurately and safely
- Perform timely patient assessments and monitor vital signs
- Document patient status, interventions, and responses in the electronic health record
- Coordinate with nurses, therapists, and physicians to ensure continuity of care
- Assist with wound care, basic procedures, and daily living needs as appropriate
- Educate patients and families about care plans and discharge instructions
- Advocate for patient safety and quality outcomes on every shift
Qualifications
- Current Licensed Practical Nurse (LPN) license in New Mexico, or eligible to obtain one
- Completion of an accredited practical nursing program or equivalent
- Basic Life Support (BLS) certification required prior to start
- Previous LPN experience in a long-term care or rehab setting preferred
- Strong clinical judgment, excellent communication skills, and a team-oriented attitude
Education
- Graduation from an accredited practical nursing (LPN/LVN) program
Knowledge & Skills
- Medication administration and safe documentation practices
- Basic wound care and infection control principles
- Patient assessment skills including vital signs and basic physical observations
- Familiarity with electronic health record systems and charting
- Good time management, prioritization, and teamwork skills
- Ability to interact compassionately with patients and families from diverse backgrounds
