Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in line with care plans
• Administer medications and treatments accurately and safely
• Perform timely patient assessments and monitor vital signs
• Document patient status, interventions, and responses in the electronic health record
• Coordinate with nurses, therapists, and physicians to ensure continuity of care
• Assist with wound care, basic procedures, and daily living needs as appropriate
• Educate patients and families about care plans and discharge instructions
• Advocate for patient safety and quality outcomes on every shift

Qualifications

• Current Licensed Practical Nurse (LPN) license in New Mexico, or eligible to obtain one
• Completion of an accredited practical nursing program or equivalent
• Basic Life Support (BLS) certification required prior to start
• Previous LPN experience in a long-term care or rehab setting preferred
• Strong clinical judgment, excellent communication skills, and a team-oriented attitude

Education

• Graduation from an accredited practical nursing (LPN/LVN) program

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic wound care and infection control principles
• Patient assessment skills including vital signs and basic physical observations
• Familiarity with electronic health record systems and charting
• Good time management, prioritization, and teamwork skills
• Ability to interact compassionately with patients and families from diverse backgrounds
