Roles & Responsibilities

• Provide compassionate, hands-on patient care and build trusting relationships with residents
• Administer medications and treatments accurately and safely according to orders
• Monitor and document vital signs, responses to interventions, and changes in condition
• Collaborate with RNs, therapists, and interdisciplinary team members to coordinate care
• Maintain clear, timely charting and ensure regulatory documentation is complete
• Assist with admissions, discharges, and patient transfers as needed
• Support residents with activities of daily living while promoting independence and dignity
• Contribute to a positive, resident-centered environment and participate in quality improvement

Qualifications

• Current Licensed Practical Nurse (LPN) license — ready to practice
• Basic Life Support (BLS) certification or willingness to obtain promptly
• Comfortable working evening/night shifts and the ability to work Sundays, Tuesdays and Fridays as scheduled
• Professional, patient-focused attitude with strong communication and teamwork skills
• Experience in long-term care, rehab, or skilled nursing is a strong plus

Education

• Graduate of an accredited LPN/vocational nursing program (diploma or certificate)

Knowledge & Skills

• Solid medication administration and documentation practices
• Good assessment skills for monitoring patient status and identifying changes
• Familiarity with electronic health records and timely charting
• Ability to prioritize tasks and manage time during busy shifts
• Strong interpersonal skills for working with residents and families
• Infection control, safety protocols, and adherence to facility policies
