Roles & Responsibilities

• Administer medications and treatments safely and accurately according to facility protocols
• Perform thorough assessments and monitor residents' conditions, communicating changes promptly
• Document care clearly and timely in the patient record to support continuity of care
• Collaborate with RNs, physicians, and interdisciplinary team members to develop and follow individualized care plans
• Support activities of daily living and provide compassionate assistance to residents with dignity
• Respond to emergencies calmly, following facility policies and ensuring resident safety
• Participate in shift handoffs and contribute to a positive, team-oriented work environment

Qualifications

• Valid Rhode Island Licensed Practical Nurse (LPN) license required
• Current BLS/CPR certification required or able to obtain prior to start
• Comfortable working the required schedule: Sundays and Tuesdays as double shifts and Fridays as single shifts
• Ability to provide safe, patient-centered care in a skilled nursing or long-term care setting
• Excellent communication, teamwork, and time management skills
• Passion for compassionate resident care and a professional attitude

Education

• LPN diploma or certificate from an accredited program and active state licensure

Knowledge & Skills

• Medication administration and basic IV/SC/IM knowledge appropriate to LPN scope
• Vital signs monitoring, wound observation, and basic dressing care
• Accurate charting and familiarity with electronic health records or paper charting systems
• Infection control principles and safe patient handling techniques
• Strong interpersonal skills for working with residents, families, and clinical teams
• Ability to prioritize tasks during busy shifts and escalate concerns appropriately
