Roles & Responsibilities

• Provide high-quality physical therapy services to residents in a skilled nursing facility
• Evaluate patient functional status and develop individualized, goal-oriented treatment plans
• Deliver skilled interventions including therapeutic exercise, gait training, balance and mobility training
• Document patient progress clearly and timely in the medical record
• Collaborate with nursing and the interdisciplinary team to coordinate care and ensure safe transitions
• Educate patients and families on safe mobility, home exercise programs and fall prevention
• Attend care planning and team meetings to advocate for patient-centered goals

Qualifications

• Licensed Physical Therapist in New Jersey or eligible for NJ licensure
• Doctor of Physical Therapy (DPT) preferred; new graduates are welcome to apply
• Comfortable working in a Skilled Nursing Facility (SNF) environment
• Basic Life Support (BLS/CPR) certification preferred or willing to obtain
• Strong clinical reasoning, documentation skills, and a collaborative mindset

Education

• Doctor of Physical Therapy (DPT) preferred; new graduates accepted

Knowledge & Skills

• Assessments and treatment planning for balance, gait, transfers and mobility
• Skilled use of therapeutic modalities and assistive device training
• Knowledge of skilled nursing workflows and interdisciplinary collaboration
• Clear, concise documentation and electronic health record familiarity
• Patient education, safety assessments and fall prevention strategies
• Ability to prioritize caseloads and manage time effectively
