Roles & Responsibilities

• Conduct thorough psychosocial assessments to understand each patient’s strengths and needs
• Develop individualized treatment plans that are practical, evidence-based, and person-centered
• Deliver evidence-based therapeutic interventions for depression, anxiety, trauma, substance use, and adjustment disorders
• Collaborate closely with primary care providers, nurses, and behavioral health colleagues to coordinate whole-person care
• Provide care across the lifespan with sensitivity to rural and underserved community needs
• Maintain timely, accurate clinical documentation that supports care continuity and compliance
• Coordinate referrals and community resources to ensure patients get the support they need
• Participate in team meetings and quality improvement efforts to enhance patient outcomes

Qualifications

• Active LISW license in Iowa (required)
• Master’s degree in Social Work (MSW) from an accredited program (required)
• Minimum of 2 years of clinical social work experience preferred
• Experience working in rural or medically underserved settings is a strong plus
• Comfort working as part of an integrated care team and coordinating with primary care
• Strong interpersonal, organizational, and documentation skills

Education

• Master’s degree in Social Work (MSW) from an accredited program

Knowledge & Skills

• Psychosocial assessment and clinical decision-making
• Treatment planning and individualized care approaches
• Evidence-based therapeutic interventions (CBT, trauma-informed care, etc.)
• Behavioral health treatment across depression, anxiety, trauma, and SUD
• Care coordination and integrated care collaboration
• Clinical documentation best practices and compliance
• Ability to engage patients across the lifespan with cultural sensitivity
• Strong communication and organizational skills
