Roles & Responsibilities

• Perform thorough patient assessments and document findings to inform care plans
• Administer medications and treatments accurately and safely
• Provide comprehensive ADL assistance to support resident dignity and comfort
• Manage gastrostomy tube care, urinary catheter care and tracheostomy care as assigned
• Deliver nebulized treatments, CPAP support and wound VAC care when required
• Admit and discharge residents with attention to detail and clinical accuracy
• Communicate pertinent clinical information promptly to MDs or NPs
• Collaborate with the interdisciplinary team to ensure continuity of care

Qualifications

• Current Licensed Practical Nurse (LPN) credential in good standing
• Experience or comfort working in long term care settings and with complex residents
• Demonstrated skills in medication administration, wound care, tracheostomy and gastrostomy tube care
• Ability to perform patient assessments, ADL assistance, and document care accurately
• Flexible availability, including weekends and holidays; ability to adapt to varied schedules
• Proof of flu vaccination and completion of employee health clearance prior to contract start

Education

• Graduation from an accredited practical nursing program and current LPN licensure

Knowledge & Skills

• Patient assessments and clinical documentation

• Gastrostomy tube care and urinary catheter care
• Tracheostomy care, nebulized treatments and CPAP support
• Wound VAC care and general wound management
• Admitting/discharging procedures and care coordination
