Roles & Responsibilities

• Provide skilled physical therapy assessments and individualized treatment plans in patients' homes
• Deliver hands-on interventions and therapeutic exercises to improve mobility, function, and safety
• Document patient encounters accurately and timely (Epic experience preferred but not required)
• Coordinate care and communicate progress with interdisciplinary team members and patients' caregivers
• Educate patients and families on home program exercises, fall prevention, and adaptive strategies
• Maintain compliance with agency policies, complete required Trinity competency testing, and support quality initiatives

Qualifications

• Valid Physical Therapy license (ability to verify license/certification is required)
• BLS certification (required)
• Experience in Home Health physical therapy preferred
• Epic experience preferred but not required
• Willingness to complete Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Ability to pass required health screenings and background verifications listed in the checklist

Knowledge & Skills

• Strong clinical assessment and treatment planning skills in the home health setting
• Skilled in gait training, balance/fall prevention, transfers, and ADL-focused interventions
• Comfortable with documentation best practices; Epic charting experience a plus
• Excellent communication skills with patients, families, and interdisciplinary teams
• Time management and organization for efficient home visit scheduling
• Knowledge of home safety evaluation and adaptive equipment recommendations
