Roles & Responsibilities

• Provide skilled physical therapy interventions in patients' homes, focusing on mobility, function, and safe discharge plans
• Perform thorough evaluations and develop individualized treatment plans that make an everyday difference
• Document care accurately and timely using HCHB to keep the care team aligned
• Educate patients and caregivers on home safety, transfers, assistive devices, and therapeutic exercises
• Collaborate with nurses, therapists, and case managers to coordinate patient-centered care
• Track progress, adjust treatment goals, and communicate outcomes to ensure continuity of care

Qualifications

• Active state Physical Therapy license (required prior to start)
• Minimum 2 years of clinical experience; home health therapy experience preferred
• Experience with HCHB documentation strongly preferred
• Comfortable providing care in patients' homes and working independently
• Valid driver's license and reliable vehicle for patient visits
• Ability to pass background check and drug screening as required

Education

• Entry-level Physical Therapy degree (DPT preferred) or equivalent clinical education

Knowledge & Skills

• Skilled in patient assessment, gait and balance training, and mobility interventions
• Knowledge of home safety assessments and adaptive equipment recommendations
• Proficient documentation skills, especially with HCHB
• Strong communication and patient education skills
• Time management and caseload prioritization for efficient home visits
• Collaborative approach to interdisciplinary home health care
