Roles & Responsibilities
- Coordinate and lead bedside case management to ensure smooth patient transitions
- Develop and manage individualized discharge plans with patients and families
- Document patient progress and care plans accurately in the EMR (Meditech/Midas)
- Collaborate with physicians, social workers and multidisciplinary teams to drive patient-centered care
- Facilitate post-acute care referrals and follow-up to prevent readmissions
- Manage throughput and capacity tasks to support efficient unit flow
- Have clear, compassionate conversations with patients and families about care goals and plans
Qualifications
- Active RN license in the state of practice (required)
- Minimum 1 year of bedside case management experience
- Experience with Meditech, HCA or Midas EMR systems
- Knowledge of post-acute care pathways and discharge planning
- Comfortable handling difficult conversations and advocating for patient needs
- Strong communication and collaborative skills for working with interdisciplinary teams
- Willingness to work rotating weekends every other weekend
Education
- Nursing diploma, ADN or BSN as required for RN licensure

- Clinical documentation and care plan development
- Patient and family communication, including difficult conversations
- Ability to prioritize and manage multiple complex cases
