Roles & Responsibilities

• Coordinate and lead bedside case management to ensure smooth patient transitions
• Develop and manage individualized discharge plans with patients and families
• Document patient progress and care plans accurately in the EMR (Meditech/Midas)
• Collaborate with physicians, social workers and multidisciplinary teams to drive patient-centered care
• Facilitate post-acute care referrals and follow-up to prevent readmissions
• Manage throughput and capacity tasks to support efficient unit flow
• Have clear, compassionate conversations with patients and families about care goals and plans

Qualifications

• Active RN license in the state of practice (required)
• Minimum 1 year of bedside case management experience
• Experience with Meditech, HCA or Midas EMR systems
• Knowledge of post-acute care pathways and discharge planning
• Comfortable handling difficult conversations and advocating for patient needs
• Strong communication and collaborative skills for working with interdisciplinary teams
• Willingness to work rotating weekends every other weekend

Education

• Nursing diploma, ADN or BSN as required for RN licensure
