Roles & Responsibilities

• Evaluate and assess residents’ functional abilities and rehabilitation needs with compassion and clinical clarity
• Develop and implement individualized treatment plans that restore independence and improve quality of life
• Provide skilled OT interventions focused on ADLs, mobility, strength, coordination, and cognitive retraining
• Collaborate closely with PT, SLP, nursing, and case management to create cohesive care plans
• Maintain accurate and timely documentation in compliance with facility and regulatory guidelines
• Educate patients and caregivers on therapeutic techniques and home safety strategies to support lasting outcomes

Qualifications

• Active California Occupational Therapist license (required)
• Graduate of an accredited Occupational Therapy program
• Previous SNF or geriatric experience preferred
• Strong communication and documentation skills — you write clearly and communicate even better
• Ability to work independently in a PRN capacity and adapt to a dynamic SNF environment
• Current CPR certification preferred

Education

• Graduate of an accredited Occupational Therapy program (required)

Knowledge & Skills

• Skilled in occupational therapy assessments and intervention planning
• Experience with ADLs, mobility training, strength and coordination interventions
• Familiarity with cognitive retraining approaches for geriatric populations
• Excellent documentation practices and regulatory compliance awareness
• Strong interdisciplinary collaboration and patient/caregiver education skills
• Clear, empathetic communication tailored to older adults and their families
