Roles & Responsibilities

• Perform comprehensive physical therapy evaluations and document findings clearly
• Develop and implement individualized plans of care focused on patient recovery and function
• Collaborate closely with PT/OT/SLP teammates and nursing to coordinate patient-centered care
• Participate in rounds and care conferences to contribute to discharge planning and progress updates
• Provide treatment across settings including patient rooms, hallways, outdoor areas, and the rehab gym
• Use Cerner and NetHealth for accurate documentation and scheduling
• Float between acute/swing, nursing home, and outpatient services as needed to meet patient demand

Qualifications

• Active NC Physical Therapy license (required)
• Current CPR certification (required)
• Minimum 2 years of clinical experience; preference for clinicians with 3+ years in a medical setting
• Experience in hospital and/or nursing home settings is a must
• Comfortable working with typical rehab equipment and in multiple care settings

Knowledge & Skills

• Skilled in physical therapy evaluations, plan of care development, and progress documentation
• Experience with Cerner and NetHealth documentation systems
• Strong communication and teamwork skills for interdisciplinary collaboration
• Ability to manage 1:1 patient care and adapt treatment across acute, skilled nursing, and outpatient settings
• Good clinical judgment for discharge planning and equipment recommendation
