Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Perform comprehensive physical therapy evaluations and document findings clearly
- Develop and implement individualized plans of care focused on patient recovery and function
- Collaborate closely with PT/OT/SLP teammates and nursing to coordinate patient-centered care
- Participate in rounds and care conferences to contribute to discharge planning and progress updates
- Provide treatment across settings including patient rooms, hallways, outdoor areas, and the rehab gym
- Use Cerner and NetHealth for accurate documentation and scheduling
- Float between acute/swing, nursing home, and outpatient services as needed to meet patient demand
Qualifications
- Active NC Physical Therapy license (required)
- Current CPR certification (required)
- Minimum 2 years of clinical experience; preference for clinicians with 3+ years in a medical setting
- Experience in hospital and/or nursing home settings is a must
- Comfortable working with typical rehab equipment and in multiple care settings
Knowledge & Skills
- Skilled in physical therapy evaluations, plan of care development, and progress documentation
- Experience with Cerner and NetHealth documentation systems
- Strong communication and teamwork skills for interdisciplinary collaboration
- Ability to manage 1:1 patient care and adapt treatment across acute, skilled nursing, and outpatient settings
- Good clinical judgment for discharge planning and equipment recommendation
