Roles & Responsibilities
- Assess patients and caregivers using the nursing process to create individualized, evidence-based plans of care
- Complete OASIS and other clinical documentation accurately and on time to meet federal and agency requirements
- Serve as Case Manager for assigned patients, proactively managing caseload and visit timing to meet productivity standards
- Implement medical plans of care and nursing interventions to ensure safe, effective home health services
- Perform high-tech skills as needed — IV use and maintenance, wound VAC management, and complex wound care
- Maintain strict infection control standards across the plan of care and during all visits
- Coordinate care with LPNs, interdisciplinary team members, physicians, and vendors to support patient goals
- Provide clear, compassionate patient and caregiver education around meds, treatments, discharge planning, and reportable conditions
- Complete required agency, state and federal forms (HHABN, Medicare Notice of Non-Coverage) accurately and timely
- Participate in IDT meetings and peer review audits, contributing constructive feedback and continuous improvement
Qualifications
- Active Registered Nurse (RN) license — able to practice in Vermont (VT) or be eligible to obtain one promptly
- OASIS experience required — demonstrated proficiency with OASIS documentation and regulations
- Home health nursing experience and strong case management skills
- Comfortable with IV therapy, wound VAC, and complex wound care (or willingness to upskill quickly)
- Current BLS (Basic Life Support) certification preferred/required per employer policy
- Strong clinical documentation skills and familiarity with McKesson or similar EHR workflows
- Excellent communication and ability to collaborate with interdisciplinary teams
- Reliable transportation and ability to travel locally to patient homes
Education
- Nursing degree (ADN or BSN preferred) — active RN license required
Knowledge & Skills
- Nursing assessment and care plan development
- OASIS documentation and timeliness expectations
- Clinical documentation best practices and McKesson workflows
- Case management and visit/time productivity standards
- Infection control procedures and application in home settings
- Patient and caregiver education techniques
- High-tech skills: IV maintenance, wound VAC, complex wound care
- Understanding of HHABN, Medicare Notice of Non-Coverage and related forms
- Ability to coordinate discharges/transfers and communicate across disciplines
