Roles & Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans for home health patients
• Deliver skilled therapeutic interventions during home visits to improve mobility, function, and safety
• Document visits and clinical progress accurately and timely using HCHB
• Coordinate care with nurses, hospice team members, and other interdisciplinary partners to optimize patient outcomes
• Educate patients and caregivers on home exercise programs, fall prevention, and safe functional mobility
• Manage a caseload efficiently while maintaining high-quality, patient-centered care during each visit
• Represent the agency professionally in the community and maintain patient confidentiality and compliance

Qualifications

• Licensed Physical Therapist (PT) eligible to practice in North Carolina
• Minimum 2 years of clinical experience; home health experience is required for this role
• Comfortable and experienced with home health patient visits and working independently in the field
• Experience documenting in HCHB or willingness to quickly adopt the system
• Reliable vehicle for patient visits and valid driver’s license

Knowledge & Skills

• Strong assessment skills for mobility, balance, gait, and functional capacity
• Skilled in designing and progressing home-based therapeutic programs
• Excellent written documentation and clinical reasoning using HCHB
• Solid communication and patient/caregiver education abilities
• Time management and route planning for efficient daily patient visits
• Ability to collaborate with interdisciplinary teams including hospice and nursing staff
