Roles & Responsibilities

• Assess patients in the home setting and perform thorough OASIS assessments to support high-quality care
• Develop individualized care plans and coordinate services across the care team
• Manage case load proactively to ensure timely visits, follow-up, and documentation
• Document visits clearly and comprehensively in the EMR to meet regulatory and agency standards
• Educate patients and families on disease management, medications, and home safety
• Collaborate with physicians, therapists, and social services to optimize patient outcomes
• Advocate for patient needs and connect them with community resources
• Ensure compliance with agency policies, OASIS protocols, and quality initiatives

Qualifications

• Active RN licensure (able to practice in Vermont; multistate license helpful if applicable)
• Home health nursing experience with documented OASIS expertise (OASIS experience required)
• Case management experience in home health settings (required)
• Travel nursing experience is preferred — experienced travelers strongly encouraged to apply
• Ability to commit to the contract period with no requested time off during the assignment
• Strong clinical assessment, critical thinking, and time-management skills
• Reliable transportation and ability to travel between patient homes

Education

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Proficient in OASIS assessments and home health documentation
• Skilled in care planning, patient education, and interdisciplinary coordination
• Comfortable with clinical decision-making in community settings
• Effective communicator with patients, families, and clinicians
• Organized caseload management and prioritization abilities
• Familiarity with home health regulatory and quality requirements
