Roles & Responsibilities
- Assess and evaluate residents to develop individualized treatment plans that restore function and independence
- Implement therapeutic interventions and progress care plans with clear, patient-centered goals
- Document treatment sessions thoroughly and timely to ensure continuity of care and regulatory compliance
- Collaborate with nursing, OT, speech therapy and other team members to optimize patient outcomes
- Educate residents and families on home exercise programs, safety strategies, and discharge planning
- Monitor and report changes in patient status, adjusting plans of care as needed
Qualifications
- Active Texas Physical Therapy license or eligibility to obtain licensure in Texas
- Doctor of Physical Therapy (DPT) preferred; Bachelor or Master in Physical Therapy considered
- Current CPR/BLS certification (or willing to obtain)
- Prior experience in skilled nursing, inpatient rehab, or long-term care preferred
- Strong documentation skills and ability to work collaboratively in an interdisciplinary team
Education
- Doctor of Physical Therapy (DPT) preferred; Bachelor or Master of Physical Therapy acceptable
Knowledge & Skills
- Proficient in patient assessment, therapeutic exercise, mobility and balance training
- Knowledge of rehabilitation techniques for skilled nursing and post-acute populations
- Comfortable with electronic documentation and clinical outcome measurement tools
- Excellent communication skills and patient-family education ability
- Ability to prioritize caseloads and manage time effectively in a fast-paced setting
