Roles & Responsibilities
- Provide compassionate, hands-on nursing care to patients in the assigned unit
- Administer medications and treatments per physician orders and facility policy
- Perform timely and accurate documentation in the electronic health record
- Monitor patient status, report changes, and participate in care planning
- Collaborate with nurses, therapists, and physicians to deliver coordinated care
- Assist with admissions, discharges, and safe patient transfers
- Promote infection prevention practices and maintain a safe patient environment
Qualifications
- Active Licensed Practical Nurse (LPN) license (PA license preferred based on location)
- Current BLS/CPR certification or willingness to obtain upon hire
- Up-to-date immunizations and ability to pass required health screenings
- Willingness to pass background and drug screening as required by the facility
- Clinical experience in long-term care, rehab, or similar settings is a plus
Education
- Completion of an accredited Practical Nursing program (diploma or certificate)
Knowledge & Skills
- Strong clinical assessment and basic nursing skills
- Medication administration and documentation accuracy
- Comfortable using electronic health record systems
- Excellent communication and patient advocacy skills
- Ability to work collaboratively in a fast-paced clinical team
- Good time management and prioritization under varying patient loads
