Roles & Responsibilities
- Deliver skilled therapeutic interventions under the direction of a physical therapist to improve patient mobility and independence
- Assist with patient assessments, implement treatment plans, and track progress toward functional goals
- Provide safe and effective patient transfers, gait training, and therapeutic exercise instruction
- Document patient care accurately and timely in the facility EMR and communicate changes to the supervising PT
- Collaborate with nurses, OT, SLP, and other team members to create cohesive, patient-centered care plans
- Educate patients and families on home exercise programs, safety, and strategies to maximize independence
Qualifications
- Current Physical Therapist Assistant license or eligibility for licensure in South Dakota
- Graduate of an accredited PTA program (required)
- Basic Life Support (BLS/CPR) certification (or willing to obtain prior to start)
- Comfortable working in a skilled nursing/long-term care setting — experience preferred
- Strong communication skills and a team-first attitude
- Ability to pass facility background checks and meet onboarding requirements
Education
- Associate degree from an accredited Physical Therapist Assistant program
Knowledge & Skills
- Knowledge of therapeutic exercise, modalities, and mobility training
- Skilled in safe patient handling and transfer techniques
- Documentation skills and familiarity with electronic medical records
- Ability to follow care plans and modify interventions under PT supervision
- Good clinical judgment, time management, and patient education skills
