Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents as an LPN
- Administer medications and treatments accurately and safely
- Perform routine assessments and report changes in condition promptly
- Document care thoroughly in the electronic health record with attention to detail
- Collaborate with nurses, providers, and care team members to support resident goals
- Support resident comfort and dignity through person-centered care practices
- Assist with admissions, discharges, and care transitions as needed
Qualifications
- Current and unrestricted LPN licensure in Pennsylvania
- Basic Life Support (BLS/CPR) certification or willingness to obtain
- Recent clinical experience in long-term care, post-acute, or similar setting preferred
- Ability to pass background check and required health screenings
- Strong communication skills and a teamwork-first attitude
- Comfort working in a per-diem model with flexible availability
Education
- Completed LPN diploma or vocational nursing program
Knowledge & Skills
- Medication administration and documentation best practices
- Basic nursing assessments and vital signs interpretation
- Skilled in wound care, ADL assistance, and resident mobility support
- Familiarity with electronic health record documentation
- Ability to prioritize tasks in a fast-paced care environment
- Strong interpersonal skills for working with residents and families
