Roles & Responsibilities
- Provide compassionate, patient-centered nursing care in long-term care and home health settings
- Perform thorough assessments and develop individualized care plans
- Administer medications and treatments safely and in compliance with policies
- Document care accurately and timely in the clinical record
- Coordinate with interdisciplinary teams to ensure continuity of care
- Educate patients and families about diagnoses, treatments, and self-care
- Adhere to infection control practices, immunization, and safety protocols
- Participate in Trinity-specific competency testing and skills validation
Qualifications
- Current RN license with verification (Connecticut license expected or ability to obtain)
- BLS certification required
- Completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist as requested
- Respirator fit test and Trinity respirator evaluation required
- Current immunizations and health screenings (2-step PPD or IGRA, MMR, Hep B, Varicella, Tdap, COVID-19, annual flu) and 10-panel drug screen
- Experience in long-term care or home health preferred
- Strong communication, time management, and clinical assessment skills
Knowledge & Skills
- Clinical assessment and care planning for LTC and home health populations
- Medication administration and IV/PO management
- Wound care and chronic condition management
- Electronic medical record documentation and charting accuracy
- Infection prevention and control practices
- Patient and family education with a focus on safety and independence
- Ability to work independently in community or home settings
