Roles & Responsibilities

• Provide compassionate, resident-centered care in a long-term care setting
• Administer medications and treatments accurately and document thoroughly
• Perform routine assessments and report changes in condition to the care team
• Collaborate with nurses, physicians and support staff to coordinate care plans
• Assist with daily living activities while promoting dignity and independence
• Maintain accurate documentation in the medical record and follow policies
• Support infection control, safety protocols and a positive resident environment

Qualifications

• Current LPN license in good standing (New York licensure required or eligible)
• Recent experience in long term care or skilled nursing preferred
• Current BLS/CPR certification
• Strong communication and teamwork skills — you’ll be part of a close-knit crew
• Ability to manage documentation, follow care plans and prioritize tasks
• Up-to-date immunizations and ability to meet facility onboarding requirements

Education

• Completion of an accredited Practical Nursing program and LPN licensure

Knowledge & Skills

• Knowledge of long-term care practices, resident assessment and ADL support
• Medication administration and safe documentation practices
• Familiarity with infection prevention and safety protocols
• Comfortable using electronic health records and point-of-care charting
• Good clinical judgment and ability to escalate changes in resident condition
• Strong interpersonal skills for working with residents, families and staff
