Roles & Responsibilities
- Provide compassionate, resident-centered care in a long-term care setting
- Administer medications and treatments accurately and document thoroughly
- Perform routine assessments and report changes in condition to the care team
- Collaborate with nurses, physicians and support staff to coordinate care plans
- Assist with daily living activities while promoting dignity and independence
- Maintain accurate documentation in the medical record and follow policies
- Support infection control, safety protocols and a positive resident environment
Qualifications
- Current LPN license in good standing (New York licensure required or eligible)
- Recent experience in long term care or skilled nursing preferred
- Current BLS/CPR certification
- Strong communication and teamwork skills — you’ll be part of a close-knit crew
- Ability to manage documentation, follow care plans and prioritize tasks
- Up-to-date immunizations and ability to meet facility onboarding requirements
Education
- Completion of an accredited Practical Nursing program and LPN licensure
Knowledge & Skills
- Knowledge of long-term care practices, resident assessment and ADL support
- Medication administration and safe documentation practices
- Familiarity with infection prevention and safety protocols
- Comfortable using electronic health records and point-of-care charting
- Good clinical judgment and ability to escalate changes in resident condition
- Strong interpersonal skills for working with residents, families and staff
