Roles & Responsibilities

• Provide high-quality outpatient physical therapy evaluations, treatment plans and hands-on interventions
• Deliver skilled rehabilitation care that improves patient function, mobility and independence
• Collaborate with clinic staff, nurses and other providers for coordinated patient care across OP, limited HH and IP settings
• Document treatments and progress accurately in the electronic medical record and maintain clear communication with patients and families
• Participate fully in the 4-week onboarding process to learn clinic workflows and ensure continuity of care
• Educate patients and caregivers on home programs, safety and strategies to optimize long-term outcomes

Qualifications

• Current, unrestricted Physical Therapy license for Minnesota or eligibility to obtain one quickly
• Clinical experience in outpatient physical therapy; experience with home health and inpatient settings is a plus
• Comfortable with hands-on therapy, exercise prescription and patient education
• Current CPR/BLS certification or willingness to obtain upon hire
• Ability to complete a 4-week onboarding and integrate into a close-knit community clinic
• Strong communication skills and a patient-first, community-minded attitude

Education

• Graduation from an accredited Physical Therapy program (DPT preferred or equivalent)

Knowledge & Skills

• Outpatient therapy evaluation and treatment planning
• Rehabilitation techniques for musculoskeletal and functional conditions
• Home health and inpatient consults and transitions of care
• Therapeutic exercise, manual therapy, gait and balance training
• Patient education and home exercise program development
• Accurate clinical documentation and familiarity with EMR systems
• Time management and caseload prioritization in a busy clinic setting
