Roles & Responsibilities

• Provide skilled physical therapy evaluations and develop individualized treatment plans that restore function and improve quality of life.
• Deliver hands-on manual therapy and therapeutic exercise interventions tailored to each patient’s goals.
• Monitor and document patient progress accurately in the medical record and adjust care plans as needed.
• Collaborate with clinic staff and other healthcare providers to ensure coordinated, patient-centered care.
• Educate patients and families on home exercise programs, injury prevention and community resources.
• Support clinic operations by maintaining timely documentation, following compliance standards, and participating in team meetings.
• Contribute to a welcoming clinic culture that promotes compassionate, community-focused rehabilitation.

Qualifications

• Licensed Physical Therapist in Minnesota or eligible to obtain MN licensure prior to start.
• Graduate of an accredited Physical Therapy program (DPT preferred).
• BLS/CPR certification (or willing to obtain upon hire).
• Comfortable with outpatient rehabilitation caseloads and hands-on treatment approaches.
• Able to complete a minimum 30-day onboarding process prior to assignment start.
• Strong communication and teamwork skills; dependable and patient-centered.

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor’s or Master’s in Physical Therapy considered.

Knowledge & Skills

• Skilled in musculoskeletal evaluation, therapeutic exercise, and manual therapy techniques.
• Proficient in gait training, balance and functional mobility interventions.
• Experience with documentation and working in electronic medical record systems.
• Ability to educate patients and families in clear, practical ways.
• Good time management and organizational skills in a busy outpatient clinic.
