Roles & Responsibilities

• Provide direct patient care through skilled evaluation and individualized treatment planning that improves mobility and quality of life
• Perform clinical screens and comprehensive evaluations to set evidence-based therapy goals
• Deliver hands-on treatment including manual manipulation, transfers, gait and balance training
• Maintain accurate, timely patient documentation and communicate treatment outcomes clearly
• Coordinate billing and reporting with the Rehab Director to ensure correct therapy claims and records
• Supervise and mentor support personnel to ensure safe, effective patient care
• Develop and present in-services and participate in interdisciplinary meetings to drive patient-centered care
• Provide family and caregiver education to support carryover of therapy goals outside of sessions
• Participate in community education and public relations activities as requested by the facility

Qualifications

• Arizona license required for submission — OR already applied with an ETA for approval (must provide ETA)
• Current certification with the American Physical Therapy Association (APTA)
• Minimum of a Bachelor’s degree in Physical Therapy
• Ability to safely perform patient handling tasks, including minimal assist transfers (able to lift/control up to 50 pounds)
• Comfort performing primarily physical tasks (approximately 90% physical activity)
• Ability to follow Medicare, facility, and Therapy Management Corporation policies and documentation standards

Education

• Bachelor's degree in Physical Therapy

Knowledge & Skills

• Direct patient care and clinical evaluation expertise
• Treatment planning and hands-on manual therapy skills
• Transfers, gait training, balance assessment and mobility assistance
• Accurate patient documentation and familiarity with billing/reporting processes
• Medicare compliance and facility policy knowledge
• Experience supervising support personnel and delivering in-service education
• Strong communication and teamwork skills for interdisciplinary coordination
• Community and family education experience a plus
